Overview and Purpose
The co-evaluation form is the final deliverable of the semester. This form helps to ensure that everyone does their fair share of work throughout the semester.

Format
You will fill out a section for each member of your group, including yourself. Be honest in your evaluations, as what you say will affect the grades of your teammates. Each team member should fill out their own evaluation, separately. The co-evaluation form can be found at this link. Email your individual form to your mentor upon completion.

Grading Rubric
We will consider each of your team members’ evaluations (including your own) when determining your grade.
